Why do patients miss appointments?

The same review lists the factors most often associated with no-shows: younger adults, lower socioeconomic status, living far from the clinic and having no private insurance. The two determinants reported most often are a long lead time between booking and appointment and a history of previous no-shows. Patients who booked far in advance and have missed before deserve extra attention.

The old method of telephone calls or messages sent manually by reception fails for three reasons: limited business hours, patients forgetting when the reminder arrives too far in advance and friction when rescheduling.

Do message reminders work?

The strongest evidence comes from a Cochrane review of eight randomised trials (Gurol-Urganci et al., 2013). Attendance was 67.8% with no reminder and 78.6% with mobile message reminders. Messages performed similarly to phone calls (80.3%) and cost less per attendance. The trials tested SMS, not WhatsApp, and the authors rate the evidence as low to moderate quality. Your clinic's result has to be measured, not assumed.

How the WhatsApp flow works

ClínicaSync automates the reminder sequence: a 24-hour WhatsApp reminder with CONFIRM or RESCHEDULE options, followed by a 1-hour reminder with the location and pre-visit guidance.

If the patient clicks RESCHEDULE, the bot offers the doctor's available slots and fills the vacated slot from the waitlist.

The confirmation reminder runs into a platform rule: the official API only allows free-form messages inside the 24-hour customer service window opened by the patient's last message. Outside it, the clinic must use message templates approved by Meta (Cloud API documentation). That is why the reminder is registered as a template with CONFIRM and RESCHEDULE buttons.

How do you measure the result in your clinic?

Before switching the automation on, record four weeks of no-show rate, split by weekday and appointment type. Then compare the same cut with reminders active. Automation reduces the rate, but the exact gain depends on volume, send time and channel read rate. The reminder must arrive early enough for the patient to rearrange.
